A Rich, Meaningful Life for Your Loved One. Peace of Mind for You.
Choosing the right memory support community for a loved one can be extremely difficult and emotionally taxing. We understand, and we’re here to help. Here are just a few things families tell us that make them feel good about choosing Rockridge:
Weekly rounds from our on-site physician and nurse practitioner who work hand-in-hand with our care team and families to ensure resident’s receive the best care. Our life enrichment director also works with the care team and families to gear activities to each resident.
Our life enrichment director works closely with our care team to create engaging activities and musical programming that bring joy by reminding residents of who they were, who they are and what they love.
Our care team holds a daily huddle to discuss each resident’s physical and mental health, make care plan adjustments, collaborate with our medical team and keep families updated. Any family member can contact any care team member anytime to ask questions and make requests.
Customized Care Packages
Like everything else at Rockridge, the Memory Support care your loved one receives is the result of a close collaboration between you, your loved one and our care team. We’ll work with you on an ongoing basis to customize one of the care packages below to make sure your loved one receives the care he or she needs. (Note that these packages are different than our Assisted Living care packages.)
Memory Support Care Packages
This package is good for residents actively involved in the completion of their bathing, dressing and grooming. They may need some assistance with a shower or other personal care up to 4 times a week. Otherwise, the residents can manage independently with assistive devices if needed.
